A basic component of our commitment to our principles is our respect for you, our customer, and the privacy of the information that you have entrusted to us.

This following describes our efforts to safeguard your privacy, the types of information that we collect, and how we use this information to serve your health care needs more completely.

Types of Information We Collect

We may collect non-public personal information about you from the following sources:

  • application, health questionnaires, satisfaction studies
  • your medical record from services being authorized
  • consumer reporting agencies

Non-public personal information is information about you that is not publicly available that we obtain in connection with providing a health care product or service to you.

For example, non-public personal information includes information regarding your medical history, current health status, types of medical procedures, and/or utilization patterns.

Types of Information We Disclose

To Non-affiliate Third Parties:

We may disclose non-public personal information about you to non-affiliated third parties as permitted by law. In certain circumstances we are permitted under law to disclose non-public personal information about you to non-affiliated third parties (third parties that are not members of our corporate family).

For Example, we may disclose non-public personal information about you to such third parties to assist us in providing routine, urgent or emergency services. Such parties would consist of hospitals, physicians, home health agencies, pharmacy benefit managers, and other subcontractors; to government entities that we may contract with or in response to subpoenas; and to consumer reporting agencies.

To Joint Marketing Partners:

We may disclose the following information to companies that perform marketing services on our behalf or to other health care providers whom we have joint marketing agreements:

  • Information we receive from you on applications, contracts, or other account forms, such as you name, address, social security number, and ID number.
  • Information about the type of benefits you are eligible for under the plan.

To Our Affiliated Health Care Agencies:

We are permitted under law to share information about our experiences or transactions with you (such as your medical status, and medical history) with companies related to us by common control or ownership (“affiliates”)

We may disclose to our affiliates the following kinds of non-public personal information about you.

  • Information we receive from you on enrollment application or other enrollment information, such as your name and address; and
  • Information about your medical status and history with us, to include your claims experience.
Our Security Procedures

We also take steps to safeguard your customer information. We restrict access to non-public personal information about you to those employees who need to know that information to provide products or services to you. We maintain physical, electronic, and procedural safeguards that comply with federal standards to guard your non-public personal information.

The protections outlined in this notice will continue even should you decide to dis-enroll or become an inactive member.
